No fixed first-aider-to-worker ratio is set by law, in either the private or public sector. Under 29 CFR 1910.151(b) (mirrored by the state plan for public employees), when no infirmary, clinic, or hospital is in near proximity, the employer must ensure that a person or persons are adequately trained to render first aid. OSHA guidance expects at least one trained responder per shift, with emergency care reachable within 3-4 minutes at high-hazard sites (up to about 15 minutes at low-hazard offices).

Federal OSHA and the Massachusetts public-sector plan accept blended training (online coursework plus an in-person hands-on skills demonstration) for workplace first aid and CPR. No ban on online training; online-only theory is discouraged only where practical skills such as CPR cannot be demonstrated.
